The most important recent advance in malignant hematology is the recognition of the prime importance of the supportive tumour cell microenvironment such as in multiple myeloma, and the need to target it in refractory diseases such as chronic lymphocytic leukemia, and to realize that it has been relatively neglected in "refractory diseases such as peripheral T-cell lymphoma".
